Thanks for the info. The reclusive Owner of Health Research may have passed on, I am not certain. But I do know that Health Research was sold to people in Washington State. I have spoke to them on the phone and have their Health Research catalog
here, it is a continuation of the former format. But they also do not publish all that the previous owner did. I have their 1998 version, here is the vital contact information if you wish to contact them:

I think that I have an offset-printed facsimile copy of this book, "The Inner Group Teachings of H.P. Blavatsky: To Her Personal Pupils", bound in spirals, that was published about 30 or 40 years ago by a small publisher, Health Research, of Mokelumne Hill, California, that specialized in reprinting out-of-copyright old books on the occult, health, and controversial subjects generally. It is not certain whether they still exist; the firm's director was a chiropractor named Wilborn.
I will see if can find it. As I remember, it appears that the original was typed with a manual typewriter, and may have been printed by means of a spirit duplicator.
