Here some short answers on your questions;

WWW was Dr. William Wynn Westcott, Coroner for North-East London, 
(born in Leamington Spa, England, 17 December, 1848 and dead in 
Durban, South Africa, 30 July 1925), Vice President of the Blavatsky 
Lodge of the TS, member of the Inner Group of the ES, Founder and 
President of the Ananda Lodge of ES in London 1893 (the only ES-
lodge that was pure male (in other words, women wasn't allowed to 
join), closed 1895), Supreme Magus of Soc.Roc. (S.R.I.A. - Societas 
Rosicruciana in Anglia), Founder of the Esoteric School of Masonic 
Research, Joint Other Chief of the Golden Dawn (whit Woodman and 
Mathers), High ranked Mason etc.

He also wrote parts of the Theosophical Glossary (his articles are 
marked WWW).

About ES and GD. This is from a footnote of the G.deP. EST Book of 
Rules (Point Loma, 1930). My copy is the Swedish translation from 
1931.

Gilbert (Theosophical History Centre, 1987).

I don't know if Countess Wachtmeister was involved, but her father 
the French Marquis de Boubel-Montpiaçon, was a Rosicrucian and 
Mason, so she was probably familiar whit this since her childhood.

Crowley's "The Voice of the Silence and Commentaries" was first 
published in "The Equinox" vol. III, no. I in 1919. It is not sure 
when he wrote it, but it was after 1905, and before 1917. It was re-
published in "The Equinox vol. IV, no. I" (subtitle; "Commentaries 
on the Holy Books and Other Papers), printed by Samuel Weiser, 1996. 
 On the titlepage we find; "LIBER LXXI, The Voice of the Silence, 
The Two Paths, The Seven Portals by Helena Petrovna Blavatsky 8°=3°, 
with a Commentary by Frater O.M. 7°=4°". The symbol after the two 
last numbers, are actually small scares, but Yahoo don't take that 
sign. "8°=3°" was in GD the Grad number that was standing for 
a "Secret Chief". 

And by the way. The reason for the book was called LIBER LXXI, was 
be course the numerical value of the Tibetan word LAM (the root to 
Lama), meaning Way or Path, is just 71.
