It is possible he could have been referring to "the great religious
doubt" which is used in Zen to smash through the mind of delusion and
attain enlightenment. I am not aware of anyone going insane but this
does test the mind and can cause headaches.
As for doubt of hte more ordinary sort, consider this statement from
Socrates (quotes by Plato in THE PHAEDRUS):
"The wise are doubtful, and I should not be singular if I, too,
doubted."
