Carlos,

"Key to Theosophy," page 300, "The Abuse of Sacred Names and Terms," 
is all I can find. For some reason I feel a bit uncomfortable each 
time the Masters are referred to by name. I prefer first initials, 
with respect and reverance. It was interesting for me to read the 
above-mentioned section of the book.
