Yes this is a good question, the lady Pam Reynold's even made the 
comment as to whether it 'was a reality or a projection'.
I suppose not being Adept's we can only make an educated guess.

The "relatives" may be entities or energies that are 'clothed' in the 
familiar forms of the persons own mental pictures and memories stored 
in there own astral body.
Or could just astral imaginings pure and simple.

The kama-loka is a very deceptive realm (theosophy suggests) to the 
uninitiated seer.
HPB wrote a lot on this type of thing with spiritualist phenomena in 
the key to theosophy.
I'll try and find some of her comments that may be relevant.

It would be interesting to see the various occult rational for what 
people may be experiencing in the NDE state.
