Eldon,
I agree. My personal expression metaphorically is this in regard to 
blaming "Organizations":

Place every man, women and child on the moon. Then return to Earth 
and 
take a leisurely tour being careful to observe what activity is 
taking place
and what is notably absent that previously occupied the artifacts of 
humanity.
It will soon become obvious what is the responsible agent, micro or 
macro
scale of magnitudes. Like Truman liked to say: "The Buck stops here".
