Source of Kwan-Yin Pledge

Although the idea is expressed throughout Mahayana writings, I
would like to know the original source of the exact wording of
the Kwan-Yin pledge. I am guessing it is from, or based on, S.
Beal or some 19th century translator's work -- but I would rather
know for sure.

Any certain knowledge out there?
